Any screening questionnaire offered on this site is a screening aid only. It does not produce a diagnosis, and its result should be discussed with a qualified clinician before you act on it.
Appointment requests
Submitting a request through this site does not confirm an appointment. An appointment exists only once the practice has confirmed it with you directly.
Text messaging
Where you consent to SMS, you may receive messages about appointments, reminders and general two-way communication. Message frequency varies. Message and data rates may apply. Reply HELP for help, or STOP to opt out. Consent to messaging is not a condition of receiving care.
